How to Enter: No purchase necessary. This sweepstakes begins on 5/8/23 at midnight Central Time and ends on 6/5/23 at 11:59 PM Central Time. You can enter this sweepstakes by the following means: Online entry: Enter by visiting www.driversafetyquiz.com during the Entry Period. Only one entry per person.
Eligibility: This sweepstakes is open only to legal residents of the United States, 18 years of age or older at the time of entry. Online entrants must have Internet access as of Monday, May 8, 2023. Employees and their immediate families, including household members, of Wisconsin Department of Transportation (WisDOT), its parent, affiliates, subsidiaries, divisions, advertising and promotion agencies are not eligible.
Conditions of Entry: If you are entering online, you must be the registered subscriber of the e-mail account by which the entry is made prior to Monday, May 8, 2023. Incomplete entries will not be accepted. All entries become the property of WisDOT. By participating, entrants agree to these Official Rules, including all eligibility requirements. By entering this sweepstakes, you consent to WisDOT’s use of your name and image for promotional purposes, including online announcements, without additional compensation, except where prohibited by law. Neither WisDOT, nor its respective advertising and promotion agencies, or any telephone network or service providers are responsible for incorrect or inaccurate transcription of entry information, or for any human error, technical malfunctions, lost/delayed data transmissions, omission, interruption, deletion, defect, line failures of any telephone network, computer equipment, software, inability to access any Web site or online service, or any other error or malfunction, or late, lost, misdirected, illegible or postage due entries.
Prize: Grand Prize: Four (4) Wilson NFL Pro-Replica Footballs (approximate retail value $40/each) personally autographed by retired NFL Player Donald Driver will be awarded. The odds of winning depend upon the number of entries received. Any federal, state and local taxes are the sole responsibility of the winner. No prize substitution, cash substitution, or assignment or transfer of prizes is permitted, except by WisDOT for reason of unavailability in which case a prize of equal or greater value will be awarded. All prizes will be awarded.
Drawing: The prize winners will be determined on or about June 9, 2023 by a random drawing from all eligible entries received by the end of the Entry Period. The random drawing shall be conducted by an independent judge whose decisions are final with respect to all aspects of the sweepstakes. The winners will be notified by telephone, mail and email, if provided. The winners will be required to sign an affidavit of eligibility/publicity and liability release, and return it properly executed within 20 days of attempted notification as a condition of awarding the prize. If prize notification is returned as undeliverable or the winner fails to return the affidavits and release as specified, the winner will be disqualified and an alternative winner will be selected. WisDOT is not responsible for technical, telephone, hardware or software malfunction, lost, interrupted, incomplete, garbled or delayed electronic communications, including the inaccurate or incorrect capture of online entry information, or any other errors of any kind, however caused, that may occur in the processing of the entries in this sweepstakes. If, in WisDOT’s opinion, there is any suspected or actual evidence of electronic or non-electronic tampering with or alteration of any portion of the sweepstakes, or if any technical difficulties compromise the integrity of the sweepstakes, WisDOT reserves the right to terminate the sweepstakes or void any or all electronic entries. Persons entering via the web (Internet) must provide a valid email address. It is the sole responsibility of the entrant to notify the sweepstakes judges in writing if the entrant changes his/her email address after entering the sweepstakes. Any attempt to deliberately damage the content or operation of this sweepstakes is unlawful and subject to legal action by WisDOT or its agents.
General: The sponsor of this sweepstakes is the Wisconsin Department of Transportation (“WisDOT”). This sweepstakes is subject to all federal, state, and local laws. Void where prohibited by law. The winner(s) agree(s) that WisDOT, its affiliates and their agencies and employees shall not be liable for injury, loss or damage of any kind resulting from participating in this sweepstakes or from the acceptance or use of any prize awarded. WisDOT reserves the right to verify eligibility qualifications of any winner. U.S. law governs this sweepstakes.
Winners List: For the name of the winner(s), after June 9, 2023, email sponsor at [email protected] or send a self-addressed stamped envelope to: Driver Safety Quiz c/o AFFIRM Agency, N28W23050 Roundy Drive, Suite 100, Pewaukee, WI 53072 and include the text: Driver Safety Quiz Sweepstakes WINNER LIST. Washington and Vermont residents need not send stamped envelope. Requests received more than sixty (60) days after the completion of the entry period will not be honored.
